Dr. David Cundiff, MD

Medical Director

Dave Cundiff, MD, MPH is an experienced leader in the field of Substance Use Disorder treatment. He works with patients suffering from Substance Use Disorder to evaluate their medication needs and prescribe treatments accordingly. In addition, he regularly participates in all-staff debriefing sessions involving peers, nurses, and other prescribers. He also reviews and advises on policies, procedures, and techniques for treating substance use disorder.

Jeanette Revay

Jeanette Revay, ARNP

Psychiatric Nurse Practitioner

I was born in Butte, Montana and grew up in Seattle, WA and Pendleton, OR. I graduated high school at Thomas Jefferson in Federal Way, WA. I graduated my first degree in nursing from Blue Mountain CC.

I started my career in the hospital working in med surg surgery, ER and OB. While going to school for my graduate program in psychosocial nursing first at St. Martin’s University and then the UW, I worked as a house supervisor and nurse manager for the sisters of Providence. I then became one of the first Nurse Assists in Surgery on just one of my entrepreneurial projects. After receiving my graduate degree, i opened a private practice which is still growing in Olympia, WA. My husband Joseph and I live in Lacey. We have 7 grown children, 17 grandchildren and 2 great grandchildren who are the center of our lives.

Angela Frye, LMHC, SUDP

Clinical Director

Angela has been dedicated to the field of recovery and healing from substance abuse since 2003. During her career journey, Angela has worked on various care models in Washington State that were dedicated to diversity and inclusion, clinical quality, transitions, and holistic personal care, which aimed to optimize health outcomes and seamless care along the healthcare continuum. Angela has experience in caring for at-risk adolescents, adults, families, and marginalized patient populations with mental illness and homelessness. Since 2013, Angela has served in several leadership roles prioritizing continual process improvement and clinical quality of care aimed at treating co-occurring clients. She loves helping her communities and industry at large to bring awareness to treating addiction and educate others about mental health. She is passionate about collaborating on new ways of working to advocate for the client and their support systems suffering from the burden of addiction and mental health issues. Angela has a Master’s in Addiction Counseling and has been a licensed mental health counselor in Washington State since 2020.

Jeremy Johansen, CDPT

Executive Director

Jeremy has been in the field of substance use treatment since August of 2018, prior to that he was in the field of emergency medicine for 24 years. After spending 10 years  as a firefighter paramedic he transitioned to the field of substance use. Jeremy has a passion for working with first responders and volunteers for Safe Call Now which is a referral line for first responders to call when they are struggling. Jeremy is working on his Bachelors in counseling with an emphasis on substance use through Grand Canyon University.

Johnny Lewis

SDPT Primary Therapist

Johnny was born in Selma, Alabama in 1953, he faced racism most of his life, but there was a positive when he got to meet Dr. King, John Lewis, Jesse Jackson, and got to March for his freedom.

Johnny came from a family of alcoholics, his father died in his 40’s and all his brothers also died from complications from alcoholism. Johnny became an alcoholic and addict in 1981. He had an opportunity to move to Washington state and start over. It took 7 years, and things got worse before they got better. In 1988 he made a decision to stop using drugs and alcohol.

He will celebrate 34 years of sobriety in 2022. He started working on the Hill Top with the Logos program in 1989 getting addicts into treatment and off the street. He went through Pierce College for his degree and has been working in the field in all modalities throughout the years. He enjoys helping those who have been broken by this disease.

Persephanee Rodriguez, SUDP

Evening IOP Counselor

Persephanee was born in Southern California right outside of Los Angeles. She moved to Spokane, Washington in November 2015 to attend Eastern Washington University. She has been in the field of counseling since February of 2017. Persephanee is currently finishing her Master’s in Clinical Mental Health Counseling. She has obtained an Associate’s degree in Criminal Justice, a Bachelor’s degree in Psychology and she completed a graduate program in Addiction Studies at Eastern Washington University in 2017. Persephanee has a passion for working with people and enjoys empowering clients to be the best they can be in their own recovery.

Duane Serrano, SUDPT

Primary Therapist

Duane was born and raised in California until moving to Washington State in 1990. Prior to entering into the Substance Abuse field, he was a construction worker until he was injured from an unexpected accident that led him to substance abuse. His passion for helping others stems from the help he received himself. As a recovering alcoholic and addict, his goal is to help break the cycle of addiction through counseling and therapy. Helping those who don’t have the strength to help themselves. He has a degree in Human Services with the focus on Chemical Dependency and is working on his Bachelor’s in Behavioral Science with a focus on Trauma. He is also an active member and player of the CSSA (Clean and Sober Softball Association) and CSNSA (Clean and Sober National Softball Association) Organizations. Where their main purpose is to have fun and stay sober doing it.

Anne Finley, SUDPT

Anne was raised right here in the beautiful Pacific Northwest, though she spent many years living in Southern California, she finally came back home to stay. She has been in the medical field for over 20 years and in the substance abuse field since September 2017 where she has done everything from intake coordinator to sponsor and manager for an OTP clinic.

Anne grew up in a family riddled with substance use. She harnesses her passion and drive to help everyone she can to understand the disease of addiction and find a healthy life in recovery. She brings out of the box thinking with excitement and energy in all that she does in order to help her clients succeed.

In her free time she can usually be found running things over and off-roading in her jeep.

Annemarie Prindle

Programs Manager

Annemarie started her journey at Bayview Recovery as a Behavioral Health Technician, she then transitioned into the Case Manager Role, and finally she has settled in as the Program Manager. She grew up in Tacoma and her battle with addiction started when she was a teenager. After multiple attempts at recovery, she finally found what worked and will celebrate 4 years in July 2023. Her two daughters are her driving force and motivation to be successful. She is currently working on her BS Business/ Marketing Degree through Southern New Hampshire University. At Bayview, she is busy learning new skills and practicing old ones. She is encouraged to grow and supported in her work. Her experiences have made her who she is today, and this allows her to identify with each and every client who arrives at Bayview. She works hand in hand with the Bayview Recovery team to create an environment that encourages recovery, healing, and independence. She is on a mission to give back what was given to her and allowed her a second chance at life.

Jordyn Lumby

Case Management

Jordyn Lumby (she/her) started working in the field of substance use treatment in 2021, a year into recovery, after completing substance use and mental health treatment at Bayview. It was an honor to come full circle and give back as an employee at Bayview Recovery Center as a Residential Technician. During her two years as a Residential Technician Jordyn enhanced her personal recovery and logistical abilities through continuing her education at Tacoma Community Collage, and as now become a certified Peer Counselor through Washington state. Jordyn now holds the position of Substance Use Case Manger at Bayview. Jordyn has taken her lived experience including her productive and joyful recovery life and married it with her professional experience, leading by example, with the hope of helping others be able to do the same. She connects with clients by working side by side as their equal and providing support that only a peer would be able to provide. Jordyn is also passionate about empathy, equality, and cultural diversity and strives to create a safe space for all who need it.

Billy Anderson

Director of Clinical Outreach

Billy has been in Recovery since January 5th of 2012. His sobriety journey started at 20 years old and He has a great passion for helping young people find recovery. He is a Certified Intervention Professional (CIP) and has been trained in Interventions by some of the best in the industry. Billy loves helping guide families through the process of getting their loved ones into treatment, as well as helping those that are suffering find the help they need.

At Bayview Recovery He is the Director of Outreach- helping establish Bayview Recovery as a top notch provider for Substance use and co-occurring treatment in the country as well as the Northwest. Since 2013, he has worked in almost every sector of recovery- from a treatment technician to executive roles. Billy has worked at detox programs, residential programs, aftercare programs, and owned and operated sober living houses. His goal is to be a resource for anyone looking for treatment and recovery services, no matter the financial or insurance situation.

Heidi Krotzer

After turning her life around and getting sober on April 6, 2009 Heidi has been sharing her recovery experience with other addicts, along with her extensive yoga training in treatment centers for years. Heidi merged the tradition of yoga philosophy and movements to help newly recovering individuals begin the connection and understand how the body plays a big part in learning to live a better life. Yoga and recovery go hand in hand to Heidi, and she is excited to help others and break the stigma of what society thinks yoga AND recovery mean. Heidi has the E-RYT500 and YACEP certifications.

Office Manager

Pashia Braunesreither is the Office Manager at Bayview Recovery. She has over 10 years of administration experience and background in medical billing. She has been working in the recovery field for two years and appreciates working in an environment focused on helping people. In her free time Pashia enjoys spending time with her family.
